Home and Community Care offers the following services :- Day Centre activities & outings Carer support and respite Transport Recreation & social activities Home Help Housekeeping and laundry Information Handyman/ gardener Assessment Home Visits Shopping Meals on Wheels The adult day care activities run every day for each target group. Esperance Home Care also offers a community nursing program, which is a maintenance role for clients with long term health issues who may need monitoring, this includes the hospital to home service - visiting clients or potential clients (post referral) in Hospital to determine what services may be needed to assist clients on discharge, clients are visited at home post discharge twice and may have more regular community nursing visits is required in the long term. Community nursing also has an ongoing support/information sharing role with clients and their Carers living with dementia, memory loss and Alzheimer's.

I've been managing to hang in there for a few months but its now bee almost 4 weeks with nothing but bread to eat. I can't afford to get to any organisation to get food parcel. I have NO friends or family, I am alone all day every day. PLEASE SOMEONE TELL ME THERE IS A PLACE THAT CAN HELP
