Fremantle Connections
This is HACC programme which is an inclusive service that connects people with a disability with their community through recreational and leisure activities of their choice.Social support and respite is offerred and aims to encourage, support and problem solve with the client. The service runs from Mon-Sun., there is no fee but clients must pay for for any out pocket expenses. This ia a one-on-one IHC service for disabled residents living in the City of Fremantle, City of Cockburn, Town of East Fremantle and Town of Mosman Park.
