A case management service that co-ordinates services to people with disabilities and frail aged people with complex needs and their carers, so they can continue living in their own home. There is an Equipment pool (for Great Lakes area ONLY, that enables people in the community to access affordable equipment for short term periods, to try before you buy or after a hospital stay.
