Greater Western Area Health Service Transitional Aged Care Program helps older people at the end of a stay in hospital, who need more time and support in a non-hospital environment to complete their recovery, optimise their functional capacity, and make decisions about their long term care arrangements. Depending on your assessed level of need, the TACP provides several or all of the following: Nursing support, low intensity therapy, personal care, case management, equipment, emergency assistance, access to normal general practitioners and pharmacy services.
