Gold Coast Horizion Foundation Two programs offered on the Coast: COAST: Community access and independent living skills programme for adults 18 - 65 years with a disability (physical and intellectual). Program is designed to get clients out and about. Axiom Family Support Program: Provides assistance and support to families who have a child or adult with a disability. Can include medical aids, respite care, in-home assistance and specialist therapy. Must have DSQ funding. Limited block funding may be available.
