Local support co-ordination is a flexible program for people with a disability, their families and carers. It aims to increase connections between people with disabilities, their families and the local community. To help people with disabilities to determine their own needs and to identify the informal and formal supports that will make a difference to their lives. Local support co-ordination is available to people who have an intellectual, physical, neurological, psychiatric or sensory disability and require significant help and support.
