Palliative Care Service Co-ordinator / Clinical nurse specialist (CNS) provides nursing consultation to community, hospital, nursing home and hostel staff eg: symptom management. Kylie Beehag (full time) Yvonne Shannon (Part Time) Liaison with interdisciplinary team:- nurses, GP, OT, clergy, etc. Including PC trained volunteers for social and emotional support. These members of the team and bereavement program is co-ordinated by Yvonne Shannon. The service helps to facilitate a choice as to site of care by:- assisting families develop the skills needed to care for patient at home. -specialised equipment provided without charges. -education to multi disciplinary team. -majority of care is co-ordinated by community nurses. -after hours service available.

I had a very tough year the last few months I had to watch my mother slowly die from breast cancer, been struggling since mums passing before I turned 50. My family doesn't talk to me after mother passed been a lot of other things on my mind in regards to my immediate family. I feel so lost and lonely don't see anything to live for even though I've got 4 grown kids and 4 grandchildren still I can't feel anything any more for anyone. I need help please
