A multidisciplinary team which assists the elderly and their families to make informed decisions about the level of care required. Youth with disabilities are sometimes eligible for assessment. Cover the following local government areas: Casey, Cardinia, Frankston and Mornington Peninsula. Referrals are accepted from any source by contacting the Access Unit on 9788 1377.
