The aim is to promote and achieve maximum independence. The program provides care and rehabilitation in a 15 bed unit and in your own home. A care plan will be developed in consultation with you and your family to establish your goals. You will have access to a case manager, specialised nursing, physiotherapist, occupational therapist, speech therapist, dietician, geriatrician and social worker.
