This service provides an assessment, treatment and referral service for people over the age of 65 who have complex mental health needs or first onset of mental illness. It facilitates access to appropriate services for the client, both internal and external to the Division of Mental Health. It provides in-home assessment and support as well as inpatient care. It also provides prevention and promotion services eg Wellness Clinics which provide screening for residents in aged care facilities to allow early identification and treatment of mental illness. Education and Support to various agencies including the wider mental health team, aged care facilities, Aged Care Assessment Team and Geriatric Medicine
