Population Health( Incorporating Health Protection Group)-SOYF Program

The Population Health Division aims to protect, promote and restore people's health. It represents the combination of sciences, skills and beliefs that are directed to the maintenance and improvement of the health of whole populations through collective actions. Stay On Your Feet WA® is a collaborative state-wide falls prevention program aimed at reducing the incidence and severity of fall-related injuries among seniors. Key areas of Stay On Your Feet WA® program activity include: collaboration and partnership development; investigations into falls risk factors and best practice risk reduction strategies; awareness raising, education and advocacy; and evaluation of program activity. Free community education sessions on staying active and mobile can be booked by senior's community groups, clubs and organisations. Population Health is made up of four Primary Health Services located in te Western, Avon, Central, Eastern and Southern areas of the Wheatbelt and one Regional Public Health Unit located in Northam. Population Health staff includes: Occupational Therapy, Physiotherapy, Speech Pathology, Dietetcs & Nutrition, Social Work, Aboriginal Health, Community Nursing, Podiatry, Health Promotions, Audiology and Clerical and customer services. The Population Health division aims to protect , promote and restore people's health. The role of the Wheatbelt Population Health is broad and covers all core functions of contemporary public health practice in Australia. Basically the structure of Population Health is that they manage the health service , public health do the plannimg and policy development and the primary health service delivers the service. The Director of Population Health is located at the Wheatbelt Public Health Unit in Northam. The current acting Director is Sean Conlan.

I'm a single mum to a 10 yr old boy, Zayden.
We live in Melton, which is in the west of Melbourne.
I urgently require assistance.
Food vouchers would be absolutely ideal and a petrol voucher.
I have to drive my son to and from school.
He is back from school camp today and my registration was due the day he left for camp. Another $100+ I just don't have.
Once my rent is paid there isn't enough left to survive.
This week is extra tough.
Once bills are direct debited I'm lucky to get food for a few days.
I have no family to get help from and zaydens dad doesn't support zayden at all.
Every time I ask for help from places locally I have to be assessed and it's so much stress going through all your troubles being advised to see financial advisers. It's so depressing going through the same thing each time.
I keep getting referred to Orange Door. They seem to help everyone else I've spoken to without having to relive and go thru your depressing present reasons why you require help quite indepth they go. Which is too stressful.
They have left me without assistance on a friday left me with no help because I'd spoken to more than 1 person there they were confused and just left me with nothing.
My phone is playing up so can't receive calls always so they just didn't send vouchers they'd told me they were sending..
It's just so stressful having to explain your story all the time.
