Pormpuraaw Aged Care Program
Service covers programs - Community Aged Care Packages, Home and Community Care and National Carers Respite Program . Coordinator from the Service does the HACC assessments together with the Senior Health Worker and a Registered Nurse from the Primary Health Centre. ACAT assessments are done via phone with the Cairns Base Hospital ACAT.
