Before you go home, staff will discuss your individual needs in order to make the transition home. The going home plan may include services from a range of programs including Allied Health, Hospital in the Home, District Nursing and/or Post-Acute Care. Patients are discharged from the hospital with Medication prescriptions & information about them (medication advice) A copy of their discharge summary (this is also sent to their GP within 24 hours of the patient's discharge), Follow up appointment(s) information with specialists and GPs, Relevant health information such as wound care precautions, diabetes management and/or other health condition education information. Post Acute Care (PAC) provides funding for short terms discharge support to those clients "at risk" of failing to recuperate successfully after discharge from an acute episode in the public hospital.

My mother is 86 years old and she has early set Demancha and there is no one that can care for her 24/7 so im looking for a nursing home that will care for her but i need to find one that she can basically look after herself but to also have assistance around her 24/7 she has been hospitalised on 2 accassions now and the doctors and physio staff does not want her to stay living in her home because the only other person that lives there is my eldest son but he has cerebral phycey so can you please help me find the right residential aged care living village in the Lavington and Albury area’s only please
