Prince of Wales Community Health Services - Joint Care Planning
Joint Care Planning (JCP) is an independent forum for frail older people,people with disabilities, their carers, advocates and service providers (SPs) to come together to review client's needs and develop package of care. JCP can: co-ordinate forum, structure and document process, act as neutral facilitator, produce a clear service plan detailing allocation of specific tasks, hold regular reviews if needed. Help: people at risk even where multiple services are involved, people with problems with service co-ordination not resolved using usual channels, clients who consistently refuse services placing them at risk in own home, people with breakdown of an existing service plan. Planning forum is held with client and/or their advocate and all relevant SPs. Problems, needs, goals are discussed, collaborative care plan is developed.

Hi my name is Emma and I am a single mum of two teenagers and I am in need of help with a food Voucher and fuel voucher. I am and have been struggling financially for quite some time now and don't like asking for help, with minimum help from centrelink due to the kids ages and not being classed as a single mum and only one shift a week at work my situation has become worse and am not breaking even to pay rent and my other bills. There is no money left over for groceries or fuel. Can someone help please?
