The role of this service is to improve quality of life and prevent re-hospitalisation of patients with Chronic Obstructive Pulmonary Disease. Referral from all health professionals. Education of clients and carers regarding symptom management: risk factors, signs and symptoms. Assists with helping clients understand their medications and to take them as prescribed. Liaison between the GP/specialist and the client to enhance communication. Participation is voluntary and at no cost to the client. Rruns twice weekly supervised exercise, education and discussion regarding diet, exercise, medications and lifestyle issues.

Can I please ask how I can access an application for accommodation in your over 55 units please. I have recently moved back to WA from working in NT. I used to stay at my mum's place but my nephew has moved in so I have no housing. I am 61 not currently working may be able to get some p/t work.
