Shoalhaven District Memorial Hospital - The Ambulatory Care Team (TACT)
Nursing service for people at home when they may otherwise be admitted to hospital, or to facilitate early discharge from hospital. Service is provided 7 days per week. Patients/clients will be referred to TACT by their local doctor. TACT work closely with your doctor, carers, community health nurses, and other community based services as required. Assists to administer intravenous antibiotics, anticoagulation therapy and wound care.

Community nurses assess client needs, provide nursing care, liaise/refer to other health skilled services and advise on services/resources available. Community nurses offer education on diabetes and resources. Referrals followed up within 24 hours or as arranged with client.Visit supplier >
Hello, I am a mum of two small children aged 1 and 3. I have been thinking of the value of community and would love to reach out to the older generation who may be missing their family or not have any. I'm not sure how to go about it. I'd love to take some time to visit or meet with a group of friendly people . I think it might bring some joy to some and my children will have the benefit of getting to know a different generation.Visit member >
