Hospital@Home is a hospital in the home program that provides acute care for patients within their own home. Home@Home is Nurse led and provides comprehensive home-based acute care for patients who would otherwise require hospitalisation. The program provides up to six days of acute home care for a range of conditions, with 24 hour-on-call care provided by the patients own GP.
