St George Aged Care Assessment Team (ACAT)

The Aged Care Assessment Team (ACAT) is a team of doctors, social workers and other health professionals who provide a thorough assessment of care needs and offer advice on suitable and available care options. ACATs can provide referrals to local community services including the Home and Community Care (HACC) program. ACATs can provide approval for and referral to, Community Aged Care Packages (CACP), designed for people with low level (hostel) care needs. This is a package of co-ordinated care services delivered in the home. ACATs can approve nursing home or hostel type care, and help arrange a place in a suitable facility, as well as for respite for carers. Assessments are valid across Australia for 12 months. Younger persons suffering age related illnesses other than dementia would be considered for assessment on a case to case basis.

The community Nursing Service offers a wide range of clinical nursing services to eligible people who have chronic conditions, who have recently been discharged from hospital and /or who have continuing support needs. Service works with a range of health and community services to promote, maintain or restore the health and well being of the individual. They also encourage individuals and the community to take responsiblity for their own health and to engage in activities to improve their health and well being. We live in Melton, which is in the west of Melbourne.

Every time I ask for help from places locally I have to be assessed and it's so much stress going through all your troubles being advised to see financial advisers. It's so depressing going through the same thing each time.
I keep getting referred to Orange Door. They seem to help everyone else I've spoken to without having to relive and go thru your depressing present reasons why you require help quite indepth they go. Which is too stressful.
They have left me without assistance on a friday left me with no help because I'd spoken to more than 1 person there they were confused and just left me with nothing.
My phone is playing up so can't receive calls always so they just didn't send vouchers they'd told me they were sending..
It's just so stressful having to explain your story all the time.

My mother is 86 years old and she has early set Demancha and there is no one that can care for her 24/7 so im looking for a nursing home that will care for her but i need to find one that she can basically look after herself but to also have assistance around her 24/7 she has been hospitalised on 2 accassions now and the doctors and physio staff does not want her to stay living in her home because the only other person that lives there is my eldest son but he has cerebral phycey so can you please help me find the right residential aged care living village in the Lavington and Albury area’s only please
