The Benevolent Society - St George and Sutherland Live at Home Program - Community Options Project
Community Options provides a case management service to assist frail aged, younger people with disabilities and their carers who have complex care requirements to remain living at home. This case management model includes comprehensive assessment, individual care planning, service coordination of external organisations and informal supports, monitoring and review. The model also has brokerage dollars to coordinate private services which may not exist in the community. SSLAHP is also involved in two pilot projects funded until June 2007 for short term Case Management and Brokerage through NSW Health called SAFTE and Department of Veterans Affairs called the Veterans Brokerage Project.
