Transition Care Program Bundaberg
ACAT assessed whilst in hospital. 70+, be medically stable and ready for discharge from hospital. This services is Commonwealth and State funded program and is designed to support older people with short-term assistance that will help them to improve their health and independence at the end of their hospital stay. The program provides older people who are eligible with a package of services that includes nursing support and/or personal care.
