Transition Care Program - Townsville
Provides support for older people with short-term assistance that will help them to improve their health and independence at the end of their hospital stay. Services provided are case management, allied health services, nursing care, medical services, personal care, domestic assistance, food services. The program can be delivered either in a home environment or in a residential setting which offers Transition Care services. An ACAT assessment is required for Transition Care.
