Transitional Support Program

Short term care for a maximum of twelve weeks for people who cannot be discharged from hospital or would require hospitalisation or institutional care without in home support services. Can provide domestic assistance, personal care, overnight support, assistance with meal preparation, respite care, assistance with shopping, transport, provision of equipment, service coordination, outreach support as well as other services as necessary to meet the person's needs. Can provide service to people who do not meet HACC requirements.

Hi i'm a 34 year old pensioner with a disability I have recently been a victim of family violence. I have 3 children. My 10yo lizard was fed to my dogs as well as all my food in the cupboard, my front door was kicked in so currently has no door knob and can't be locked, my daughter was terrorizes where he stood in her room with a knife and all our clothing and most bedding was cut up. I have spoken to centrelink who cannot help me because the perpetrators address wasn't mine, I have tried welfare agencies who for the past 3 days have been full by the time I get through on the phone and no one seems to be concerned or worried about the after effects. If anyone can please help please let me know.

hi how are you. im in WA and have just lost nearly everything. i am desperate for some financial support. i have no phone9well an old one that can recieve msgs and calls 0 but have no credit, i have no food, i am trying to put what i have left into storage but have no fuel to continue and my storage unit is almost full but its all i could afford. i am desperate for any help, i dont want cash just someway off getting petrol in my car would help me soo much.thanks
