Clients will receive ongoing support and coordination to ensure quality care in their own community or community of their choice. Service, dependant on individual needs, would provide care in ways which include but are not limited to: * Rental accommodation with full time care * Full time care in a client's home * Overnight / Part time or drop in care in rental accommodation * Overnight /Part time or drop in car in a client's own home * Shared care in shared accommodation * Access to behaviour management /therapy / medical intervention * Access to community and generic facilities * Support with living skills / behaviour management / community access. This service will support people with disabilities in the Bogan, Bourke, Brewarrina, Cobar and Warren LGAs

I have been a bit lost for 8 months.
