Albany Aged Care Assessment Team (ACAT)

Aged Care Assessment Teams have the responsibility of deciding who is eligible for nursing home, hostel or community care packages. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported independent living services, extended care services and communtiy services. ACAT is located on the second floor of Population Health Unit's Community Health Clinic. The team consists of Sally Rose(Coordinator)- Social Worker , Morag Munns - RN and Russell Harkin - OT.

When the elderly are no longer able to manage at home without assistance , the ACAT helps them , and their carers , determine what kind of care will best meet their needs. This may be in a low care or high care home or community care services(CACPs and EACH) provided to them in their own home. ACATeams may include a doctor , nurse , social worker , physiotherapist or occupational therapist. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported residential services, extended care services and communtiy services. ACATs can also approve you for Transition Care which is aimed at improving your independence and confidence after a hospital stay. Transition Care can be for up to 12 weeks provided in either a residential care facility or in your own home.

I had a very tough year the last few months I had to watch my mother slowly die from breast cancer, been struggling since mums passing before I turned 50. My family doesn't talk to me after mother passed been a lot of other things on my mind in regards to my immediate family. I feel so lost and lonely don't see anything to live for even though I've got 4 grown kids and 4 grandchildren still I can't feel anything any more for anyone. I need help please
