Aged Care Assessment Teams have the responsibility of assessing eligibility for nursing home, hostel or community residential care as well as homecare option packages. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported residential services, extended care services and communtiy services. ACATs can also approve you for Transition Care which is aimed at improving your independence and confidence after a hospital stay. Transition Care can be for up to 12 weeks provided in either a residential care facility or in your own home.

When the elderly are no longer able to manage at home without assistance , the ACAT helps them , and their carers , determine what kind of care will best meet their needs. This may be in a low care or high care home or community care services(CACPs and EACH) provided to them in their own home. I have been thinking of the value of community and would love to reach out to the older generation who may be missing their family or not have any. I'm not sure how to go about it.
