Services are available to clients of all ages illnesses and disabilities, including those with aged related conditions, intellectual disabilities, trauma following road and work accidents and those who have recently been discharged from hospital. We have considerable experience with high need clients and those with behavioural problems and demonstrate a flexible approach to clients needs. ACTIP - Aged Care Transitional Intervention Program - Taree

Case management up to 8 weeks, nursing care, loaning equipment, referral, physiotherapy and Occupational Therapy. Service assists people with the following essential criteria: *Over 65 yrs with an illness / injury (over 45 yrs if indigenous) * Under 65 yrs with a disorder which would normally affect an older person * Lives within the Greater Taree, Great Lakes and Gloucester LGA's * Would benefit from a short-term intensive period of intervention. Must also have at least one of the following: * Recent acute illness / injury resulting in reduced ability to self care temporarily * Recent exacerbation of a chronic illness resulting in reduced ability to self care temporarily * Recent surgical intervention resulting in reduced ability to self care temporarily * History of non compliance with treatment / medications * Carer is frail aged * Client is the carer of a frail aged person * Recent episodes of confusion or disorientation.

With our lockdown, the down turn of my job and not qualifying for the new government assistance, not having a vehicle to run myself around I am putting my kids ahead of my own stigma today and ask for any assistance with obtaining support with some groceries, to make matters more difficult I have a child with sever allergies to nuts, dairy and wheat and the urgent issue of epipens which I need to prioritize in having renewed - this also makes food shopping near impossible utilizing the support of local food bank and significantly more expensive. Is there any food vouchers that can be accessed online to use with the supermarkets who offer a click and collect service or home delivery?
