Transition care consists of case managed packages of care. *ComPack (Community Case Management Packages): provide short term case management and brokerage services for people with complex care needs discharged from designated hospitals such as Royal North Shore Hospital and Ryde Hospital. Case management supports the person in their home for up to 6 weeks. *SAFTE (Sub Acute Fast Track Elderly Care): supports the case of older people at home whose medical condition is starting to worsen. The package provides case management and support for up to 6 weeks. *Transpac: provides case management and services for people who are assessed as needing residential level support when leaving a public hospital and are over 70 years.

Veterans Home Care providing low level support to eligible Veterans includes domestic assistance, personal care, respite, home and garden maintenance. I’m currently in treatment at canarven street and I have been working for over a year now and it’s difficult to get to the chemist on time and to see anyone at canarven street because of my work hours, I have been put on daily pick ups until 30/11/2022 when I do a test. This was changed from weekly to daily because I came in and did a sample and it wasn’t registering a temperature or anything in my sample including methadone which is impossible so my worker assumed that I was trying it on somehow and put me on daily pick ups. It was definitely a faulty test and because I didn’t hang around to do another one, she thought the worst, I understand why but I’m working away soon and I will need a 3-4 day take out but can’t get hold of my worker no matter how many messages I leave she is always busy and never calls me back so what can I do??? I’m working away in Belfast this month

I recently had a 5 day holiday for a family get together, and I had a friend from Sydney stay in my unit to look after my cat as I could not take here with us. Sadly, one of our neighbor's put a note on her car window, that she was not allowed to park the car there, and that she had been reported to Department of Housing. the spot she parked in is a spare spot, and not parking has been officially allocated to tenants. I spoke to dept of housing about this, and was told it was quite ok for me to have a visitor stay, and for them to use that car space and that they would notify all the tenants of this. But to date nothing has happened, and I have another trip away coming up soon, and really don't want this to happen again. Any advice would be appreciated.
