Community Options is a specialist program with the core business of case management for complex, high needs clients from within the HACC target group. The majority of Community Options clients are frail aged and younger people with a disability and their carers, with complex, high level needs who would benefit from case management. Other services include Home Maintenance and modification services and limited brokerage funds to purchase services not available from mainstream HACC services. ACCESS ( Aged Care Extended Support Service) providing an extension of existing support to enable people to remain living at home.

HACC Post Acute Program packages are aimed at clients who require a high level of HACC services for a short period of time (6 weeks maximum). The services provided are flexible and will be arranged to suit client needs.

Hi,
I hope everyone is doing well. I was just wondering what the process is for applying for emergency food vouchers?
I am currently in between jobs, and my savings and centrelink are allocated to rent and medical expenses.
As of June, I'll be starting my new job but until then, I'm not sure how easy it will be to afford groceries.
