Various programs are offered, including assistance with activities of daily living; community based acute care and rehabilitation; post acute care and rehabilitation; assistance pre-admission and pre-discharge in preparing / adjusting clients home environment; palliative care, and chronic disease management in the areas of diabetes, arthritis and falls management and prevention.
