The Domiciliary Allied Health Acute Care and Rehabilitation Team provides in- home professional health services to people in the QE11 and Bayside Health Service Districts. Services include physiotherapy, occupational therapy, speech therapy, dietetics, psychology, and social work. Referrals can come from health professionals, family members and self. Various programs are offered, including assistance with activities of daily living; community based acute care and rehabilitation; post acute care and rehabilitation; assistance pre-admission and pre-discharge in preparing / adjusting clients home environment; palliative care, and chronic disease management in the areas of diabetes, arthritis and falls management and prevention.
