This service offers care in the home via Community Options Program, Community Aged Care Packages (CACP) & Extended Aged Care Packages (EACH). CACPs are individually planned & coordinated packages of care tailored to help older Australians remain living in their own homes. Funded by the federal government, CACPs are for clients who qualify for low level care. Types of services in a CACP may include: personal care, domestic assistance, meals, transport to appointments & respite care. EACH packages are for those who qualify for & require high level care & may include: registered nursing care, care by allied health professionals, personal care, transport, social support, home help, assistance with oxygen/external feeding. In both CACPs & EACHs the client must be assessed by an ACAT (Aged Care Assessment Team) to determine eligibility.

ACTIP - Aged Care Transitional Intervention Program - Taree

Case management up to 8 weeks, nursing care, loaning equipment, referral, physiotherapy and Occupational Therapy. Service assists people with the following essential criteria: *Over 65 yrs with an illness / injury (over 45 yrs if indigenous) * Under 65 yrs with a disorder which would normally affect an older person * Lives within the Greater Taree, Great Lakes and Gloucester LGA's * Would benefit from a short-term intensive period of intervention. Must also have at least one of the following: * Recent acute illness / injury resulting in reduced ability to self care temporarily * Recent exacerbation of a chronic illness resulting in reduced ability to self care temporarily * Recent surgical intervention resulting in reduced ability to self care temporarily * History of non compliance with treatment / medications * Carer is frail aged * Client is the carer of a frail aged person * Recent episodes of confusion or disorientation.
