Gold Coast Home Health Link Program is to avoid preventable visits or admission to hospital or to assist patients to leave hospital earlier. This service offers short term support only. Services can include nursing, allied health, personal care, domestic assistance, carer support and respite, child care, transport for medical appointments, short term loan of medical equiment, meal preparation, emergency accommodation and overnight support. Referral is through GP or hospital.
