Mandurah Aged Care Assessment Team (ACAT)
The Aged Care Assessment Team (ACAT) assists people aged 65 and over, and their carers to determine the type of care which will best meet their needs when they are no longer able to manage in their existing home environment. ACATs provide information on suitable care options and can assist with arranging access or referral to appropriate residential or community care. Patients who are eligible for an ACAT assessment need to:- Be Aged! 70 years or more or 50 years or older ATSI ; Be Medically Stable ; Have significant physical, medical, social or psychological care needs that cannot be met more appropriately through other services ; Be a voluntary patient ; Not be eligible for assistance through Disability Services Commission.Give consent for an ACAT assessment.

This may be in a low care or high care home or community care services(CACPs and EACH) provided to them in their own home. ACATeams may include a doctor , nurse , social worker , physiotherapist or occupational therapist. Aged Care Assessment Teams have the responsibility of assessing eligibility for nursing home, hostel or community residential care as well as homecare option packages. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported residential services, extended care services and communtiy services. ACATs can also approve you for Transition Care which is aimed at improving your independence and confidence after a hospital stay. Transition Care can be for up to 12 weeks provided in either a residential care facility or in your own home.
