Short-term care can be provided within the first four (4) weeks of discharge. Provides individually tailored packages of health and community care services for children, young people, adults, families and the elderly in order to provide additional support when needed and assist recuperation following discharge from an acute public hospital or any publicly funded emergency department or rehabilitation facility. Services include home nursing, personal care, home care and some allied health.

I had a very tough year the last few months I had to watch my mother slowly die from breast cancer, been struggling since mums passing before I turned 50. My family doesn't talk to me after mother passed been a lot of other things on my mind in regards to my immediate family. I feel so lost and lonely don't see anything to live for even though I've got 4 grown kids and 4 grandchildren still I can't feel anything any more for anyone. I need help please

I’m currently in treatment at canarven street and I have been working for over a year now and it’s difficult to get to the chemist on time and to see anyone at canarven street because of my work hours, I have been put on daily pick ups until 30/11/2022 when I do a test. This was changed from weekly to daily because I came in and did a sample and it wasn’t registering a temperature or anything in my sample including methadone which is impossible so my worker assumed that I was trying it on somehow and put me on daily pick ups. It was definitely a faulty test and because I didn’t hang around to do another one, she thought the worst, I understand why but I’m working away soon and I will need a 3-4 day take out but can’t get hold of my worker no matter how many messages I leave she is always busy and never calls me back so what can I do??? I’m working away in Belfast this month
