Transitional Care Program - Westmead
The Westmead Aged Community Based Transitional Care Program aims to provide a short term domestic support and rehabilitation program (maximum of 12 weeks) to clients in their own home and community environments. An interdisciplinary team will provide individual care needs. The team works with the client and their carer/family towards maximising independence in functional mobility, activities of daily living, & connection with their community.

I am currently on Newstart and after rent and phone credit I have under $100 to live on for a fortnight. I've been managing to hang in there for a few months but its now bee almost 4 weeks with nothing but bread to eat. I can't afford to get to any organisation to get food parcel.
