Western Australian Transitional Care Service at Stirling Aged Care
Funded by the State and Commonwealth Governments, the Service uses a flexible approach to help eligible older people, who may be at risk of prematurely moving into a hostel or nursing home, stay as independent as possible by providing short-term therapy and support. The Service is offered as: At short-term stay in a residential setting followed by short-term support at home, or; Moving home with short-term support followed by on-going support from local services if needed, or; Both short stay unit & community areas involve multi disciplinary care, such as nursing, physiotherapy, occupational therapy, speech pathology & social work. After you are referred to the Service, you will be supported and encouraged to achieve your best level of independent living. The Service is short-term. The time that you would stay in the Short Stay Unit is up to 8 weeks. The extra support at home will be available for up to a maximum of 12 weeks.
