Provides practical in home support to persons with a dementing illness and their carers, to prevent inappropriate or premature admission to residential aged care. Includes meal and medication supervision, assistance with personal care, shopping and supervised social outings. Operates in conjunction with other community support services eg. Home Care, Neighbour Aids, Meals on Wheels. Community support workers give home visits and support for carers of those with a dementia related illness.
