Provides person centred, individualised support for people in their home, community or other settings and puts the individual first. Assist people living independently in their own home, in a family home, a community setting or other places where individual supports provide opportunities for people to attain, and maintain a high quality of life in a manner that they choose. The services may have a focus on complex medical and health supports, personal development or other assistance with day-to-day activities where the person supported may require assistance - but it has at its heart the INDIVIDUAL.
