Aged Care Assessment Teams(ACAT) helps older people and their carers determine what kind of care will best meet their needs when they no longer able to manage at home without assistance. This may be residential care in a low level or high level care home or community care services be they CACPs or EACH which are provided in the home. The ACAT team may include a doctor , nurse , social worker , physiotherapist or occupational therapist.The ACAT with your permission will contact your GP to gain more information about your medical condition and history to help the assessment. ACATs provide information on suitable care options and can help arrange access or referral to appropriate residential or community care. The ACAT can also arrange respite care or assist in finding suitable Aged Care Facilities in your region. There are no charges for an ACAT assessment.

Aged Care Assessment Teams have the responsibility of assessing eligibility for nursing home, hostel or community residential care as well as homecare option packages. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported residential services, extended care services and communtiy services. ACATs can also approve you for Transition Care which is aimed at improving your independence and confidence after a hospital stay. Transition Care can be for up to 12 weeks provided in either a residential care facility or in your own home.
