Nursing service for people at home when they may otherwise be admitted to hospital, or to facilitate early discharge from hospital. Service is provided 7 days per week. Patients/ clients will be referred to TACT by their local doctor. TACT work closely with your doctor, carers, community health nurses, and other community based services as required. Clients are asked to refrain from smoking and restrict pets from the room when staff visit. All staff wear identification which has the IAHS title and logo, the staff members name and photo. Clients should check this prior to letting people into their home.
