The Community Hospital Interface Program (CHIP) screens, assesses and then co-ordinates services required by patients after hospitalisation or presentation to Emergency Department. Target group primarily aged people. An assessment will identify potential needs that may arise when discharged. CHIP nurses at QE11 Hospital, PA Hospital, Mater Mothers Hospital and Mater Adult Hospital.
