Dementia Care Programme

The Dementia Care Program is a run by a small team of community specialist nurses with experience and training in the nursing management of dementia. They will visit the client and carer in their home and do a comprehensive assessment, including daily living skills, also offers counselling, emotional support and referral to other supportive services, admission support to a day respite centre and client monitoring and ongoing evaluation. The team offers management and coping strategies, counselling, assessment of their current situation, future life planning and assistance with additional resources. The program offers families counselling and advice within a family group. The team also runs Support Groups for carers at various locations on the Coast (listed separately). Also runs the Living with Memory Loss Program (listed separately)

Community Aged Care Packages (CACPs) aims to provide a community alternative for frail older people whose dependency and complex care needs would qualify them for entry to low level residential care. The CACPs are individually tailored packages of care services to frail older people assessed as requiring a range of services in their own homes. An ACAT must have assessed all recipients as eligible. Service providers use a case management approach to develop and monitor care delivery to the client.

The Inpatient Aged Care Assessment Team consists of Medical practitioners, social workers, and other health professionals. The team assesses the individual care requirements of each person. Following the assessments, the team in consultation with the patient and family will make recommendations that may include referral to local community services, Community Aged Care Packages, coordination of alternate accommodation such as hostel type or nursing home placements.
