Vision Australia provides Community Aged Care Packages (CACPs) in Southern Metropolitan Region. CACPs are individually planned and coordinated packages of care tailored to help older Australians remain living independently in their own homes. They are funded by the Australian Government to provide some of the more complex care needs of older people. The types of services that may be provided as part of the package include: Personal care, Social support, Transport to appointments, Home help, Meal preparation, Gardening. To receive a CACPs package people need to be assessed as eligible by an Aged Care Assessment Team.
