Local Area Coordination is available to people with physical, sensory, neurological, cognitive and/or intellectual disabilities who reside in the country and who are under the age of 60 at the time they apply for LAC support. Community access service that provides a range of support, for people with a acquired brain injury between the ages of 16 -60 years. Each person is linked with a Co-worker - a staff member who can assist the person in sorting out the options they would like to pursue; and then jointly developing a plan to achieve these goals- such as to re-learn independent living skills and access recreational social or educational activities of their choice in the community. Role of the service is to improve quality of life and prevent rehospitalisation of stroke patients. Home follow up program to enhance carer quality through co-ordination of support, health care services and respite services. This service educates clients and carers regarding strokes, risk factors, signs and symptoms. Assists with helping clients to understand their medications and to take them as prescribed. The Mental Health Review Tribunal is an independent body which upholds the civil and legal rights of people who are mentally ill or mentally disordered and ensures that they receive the best possible care in the least restrictive environment. The Mandurah office is the-Partners-in-Purpose-Programme for people with disabilities who are registered and/or funded by DSC.This is also a private service.The services offered are varied to suit each individual to be able to maintain independence at home.Other service offered is out of home (usually in host-carer's home) host-family respite with overnight stays between 24-48hrs. The regions covered are from Coogee to Bunbury. The service is specifically aimed at supporting people who receive Accommodation Support Funding, Intensive Family Support funding and Alternatives to Employment funding through the Disability Services Commission. This service provides case management and brokerage support to people with a disability. The program will be offered for a period of 6 to 12 months. Referrals are received and reviewed regularly by priority of need where vacancies exist. Once referral is accepted, a case manager is appointed. An assessment is done and an individualised case plan is developed.
