Provide support to people in their own home who suffer from a mental illness. Have one unit for limited stay (two bedroom house- this is a step-down facility) up to 3 months. Clients are referred through their case managers or through GSMHS. Support activities include: independent living and household management skills; accessing community services to foster development of social skills, confidence and self-esteem. This service mainly provides outreach to people with a mental illness in their own homes, or other community residential/facility settings. In-home support is individualised around the client's own goals and lifestyle with an emphasis in learning and or maintaining skills with a view to recovery. They also offer respite in-home for the carer to have a break.They also have a community sports and recreational program for their clients which is also an out of home respite.

Adults aged 18 years and older who have a mental illness (and in special circumstances 16 - 18 years), particularly people marginalised from mainstream services including people experiencing homelessness, aboriginal and people from diverse cultural backgrounds are eligible to be referred to the units for accommodation. Crisis / Respite Accommodation at St Barts' :- The provision of accommodation for up to eight - twelve weeks for people who are experiencing a crisis, have a mental illness and who require ongoing clinical support. The Rockingham/Kwinana crisis unit extends the stay for residents for up to six months. Each facility has six single rooms and is staffed for 24 hours / 7 days a week. Residents are assisted with daily living activities. Close contact is maintained with mental health service clinical staff through caseworkers and accommodation officers.
