Community Options aim is to provide appropriate and sufficient support to enable the frail aged and people with disabilities who have complex needs to continue to live in their own home. Targets people with complex care needs & whose needs cannot be meet by existing community services and their carers. A service fee will be negotiated with each client. The caseworker works with each client to link them into appropriate services and develop a care plan tailored to meet individual needs and enhance their independence. The Host Family Program organises regular, planned home-based respite care for families who have a child (0-18 years) with a moderate to severe disability. Care is provided by trained volunteers from the community. Care is provided either: in the home of the carer or in the home of the child through community activities with the carer. Families and Volunteer Carers receive support, information and guidance from the Host Family Program Coordinator. Limits per month 48 hours for regular planned respite, 16 hours sitter, 16 hours companion.

Blacktown Respite Options

The service provides a respite options program for a limited number of families living within the Blacktown Local Government Area. The children are aged between 0-18 years, with a moderate to severe disability and having high support needs. Each child accepted on to the program is allocated funds for the financial year, which are then used to meet the family's respite needs. Each child is assigned a one to one carer for the duration of their funding. The carer is carefully selected, given the child's specific needs. HACC funded.
