When the elderly are no longer able to manage at home without assistance , the ACAT helps them , and their carers , determine what kind of care will best meet their needs. This may be in a low care or high care home or community care services(CACPs and EACH) provided to them in their own home. ACATeams may include a doctor , nurse , social worker , physiotherapist or occupational therapist. Aged Care Assessment Teams have the responsibility of assessing eligibility for nursing home, hostel or community residential care as well as homecare option packages. They do this by assessing a person's mental, physical and social situation. If residential care or community aged care packages are not appropriate they can refer people to supported residential services, extended care services and communtiy services. ACATs can also approve you for Transition Care which is aimed at improving your independence and confidence after a hospital stay. Transition Care can be for up to 12 weeks provided in either a residential care facility or in your own home.

The ACAT team may include a doctor , nurse , social worker , physiotherapist or occupational therapist.The ACAT with your permission will contact your GP to gain more information about your medical condition and history to help the assessment. ACATs provide information on suitable care options and can help arrange access or referral to appropriate residential or community care. The ACAT can also arrange respite care or assist in finding suitable Aged Care Facilities in your region. There are no charges for an ACAT assessment.

Eligibility for an ACAT Assessment Patients who are eligible for an ACAT assessment need to:- Be Aged! 70 years or more or 50 years or older ATSI (there are occasional exceptions) Be Medically Stable!!! Those patients who are receiving acute care or engaged in a rehabilitation program are not eligible . Have significant physical, medical, social or psychological care needs that cannot be met more appropriately through other services Be a voluntary patient. Not be eligible for assistance through Disability Services Commission. Give consent for an ACAT assessment.
