Transition care consists of case managed packages of care. *ComPack (Community Case Management Packages): provide short term case management and brokerage services for people with complex care needs discharged from designated hospitals such as Royal North Shore Hospital and Ryde Hospital. Case management supports the person in their home for up to 6 weeks. *SAFTE (Sub Acute Fast Track Elderly Care): supports the case of older people at home whose medical condition is starting to worsen. Funded by the federal government, CACPs are for clients who qualify for low level care. Transitional Aged Care Program (TACP) is a collaboration between Greater Western Area Health Service (GWAHS) and Baptist Community Services (BCS). The service provides up to 12 weeks support for older people who have been in hospital. While on the program, clients receive a mix of the following: Case management, therapy, care input and medical advice. TACP is aimed at older people who are at the conclusion of a hospital stay and require more time and support in a non-hospital environment to complete their restorative process.
