The Community Neurological Support Service is one of the teams within the Community Outreach and Therapy Services. CNSS provides two forms of service: 1. Intensive Multidisciplinary Home Based Rehabilitation - for clients with a recent neurological event (less than one month post event or discharge). Must have achievable goals and require input from a minimum of two therapists for a period of up to three months. 2. Four Week Stroke Education Group - for chronic stroke survivors and their carers. Participants may go on to therapy groups including an exercise group and a communication group. The service offers brief occupational therapy, physiotherapy, speech pathology, social work and dietetic intervention to people attending the group with a focus on enhancing quality of life. All clients MUST BE Medically stable, sixteen years of age or older; living at home or in a hostel on the Central Coast.

The Carers Support Group is run through the Dementia Care Programme of Area Health. The group's facilitator is a specialist in dementia nursing care. The programmes are delivered in an informal comfortable setting, bringing together carers of those clients who are suffering dementia. The purpose is for mutual support and understanding. The programme's aim is to give support, guidance and education to carers, past and present who are caring for someone who is suffering from dementia, to induce an emotional relief of anxiety within a SAFE environment. Clinics are held each Wednesday between 8:30 am and 10:30 am. BY APPOINTMENT ONLY. The clinic is attended regularly by a physiotherapist, prosthetist and doctor. For each clinic, if necessary the services of an occupational therapist or social worker can be arranged. If client feels the need to see a doctor this needs to be stated when making an appointment. The aim of the clinic is to facilitate early mobilisation of amputees involving assessment, fitting and training in care and use of prostheses, and continuing care. Clinics are also held at Wyong Hospitals Wednesday 1 pm - 3 pm (listed separately)

Provides assessment of patients exposed to various toxins, referral to a health care facility if necessary or advice on treatment in the home. Also provides advice to medical professionals on management and prognosis of poisoning cases and the treatment of bites and stings, advice on side effects and interactions of drugs, prevention of poisoning, pesticide and chemical safety also general queries relating to toxicology

The role of the Primary Health Care Nurse is to provide a day a week of service to the towns listed. She supports the visiting GP by working as Practice Nurse doing immunisation, dressing and pathology in the clinic or by doing home visits and Health Assessment for the GP in the communities. Health Promotion Activities are reguarly run in each community. Community Capacity Building to find strategies for improved health and wellbeing. This may involve submission writing for funding.
