Kwinana Home Support Group: Provide Home and Community Care (HACC) services and other supportive programs. For Frail aged and Younger people with a disability, living in the boundaries of City of Kwinana. Services are Respite Care, Domestic Assistance, Shopping Assistance, Home Maintenance, Meals, Personal care .Kwinana Home Support Services also has an opportunity shop. Visits many ACFs in the area under the Community Visitors Scheme. The respite Centre is on the same site as admin and has x4 beds and provides 24/7 EN care.There is also a Day-care centre for Seniors(not dementia specific) one day aweek where meals are provided.They have HACC-funded transport and also their own transport charged on a fees-recovery basis only that covers areas for the elderly that HACC will not fund ,i.e., medical visits and medical treatment visits like radiology or dialysis or cancer treatments etc. Also runs Kwinana Karers klub from same building that meets monthly for family carers.

The St John Community Care Branch is aimed at providing friendly support to the lonely, frail aged and disabled. Our Volunteers help meet the social, recreational and conversational needs of those less advantaged in our community. Community Care is all about bringing a little sunshine into people's lives. A friendly face and kind word can make a person's day. The value of this companionship is immeasurable, for both recipients and volunteers. St John Community Care Registered Volunteers do not wear uniforms nor are they expected to be trained in first aid. As a St John Volunteer you will be asked to set aside some time to visit, or call, someone in your community. How you spend this time together depends on you and the person you visit. You may choose to share a meal, go shopping, or simply have a chat over coffee. St John will provide reimbursements for modest expenses, such as telephone or travel, which you may incur during your visits.

Cockburn Community Care: Provides Home and Community Care (HACC) services and other supportive programs. They provide a wide range of activities and outings which are client focused to cater to the physical, social and emotional needs of all members. Services / Activities: Respite Care : both in home and out of home, to suit individual needs. Respite and Recreation Club Respite & Recreation Program for People with Special Needs(HACC) provides respite to carers of people with a disability ages 18 - 65 years, operates Sundays and Mondays, Thursdays and Friday evenings and Saturday mornings. Some of the programs that the members are involved in are: living & Arts, Music & Movement, Gardening, Restaurant preparation and planning of community fairs. In addition to the weekly programs we also offer weekend respite, and quarterly activities on Saturday evenings. Friday and Sunday groups for adults. Disco first Friday of month. To provide facilities and services that meet the individual long term care needs of people who are aged or disabled. Some of the community programs include domestic assistance, garden maintenance, personal care, host family respite, respite, social supportday care centres and a meals service. Hawthorn Respite House

Hawthorn Community Respite House is a respite facility that provides overnight care and day respite for small groups of people .Respite options include:- Short Stay Overnight Breaks - Two-day breaks (W/E only)@ $25 per day (includes meals and transport) . Other services offerred:- Men's Club - Tues 10.30am - 3.30pm, support for up to 8 men in the earlier stages of dementia including social and recreational activities, $10 per day. Activ's People, Skills and Community (formerly ATE) Service offers a choice of community and centre based activities in an environment that enables participants to form friendships and learn new skills. It provides support to people with disability who are not able to maintain full time employment. The day care centre's aim is to encourage involvement in everyday community life whilst promoting skill development, enjoyment and satisfaction. Mercy Aged Care has been providing a range of services that will to assist frail aged and younger disabled to remain living at home. HACC eligible clients in the Rockingham, Kwinana, Cockburn and Melville LGAs. The HACC services provided are: assessment, domestic assistance, home maitenance, personal care, respite care and social support. Community Aged Care Packages: This specific service is a coordinated package of home care services backed by a 24-hour emergency assistance line. Must have an ACAT. CACPs provide an alternative to low level residential care for the frail aged. CACPs may include assistance with a range of the following personal care, meal preparation, emotional support, cleaning and laundry, shopping. There are x72 CACPs(Gen) offerred in the LGAs of East Freo, Freo , Kwinana, Rockingham and Mandurah. RN employed by the Neurological Council of WA (Inc) as the Neurological Nurse Specialist for the Great Southern Region. Give specific nursing advice, care and support to people with a Neurological condition. This care/support includes compliance with a treatment regime prescribed by their GP, Neurologist and other Health Professionals. A nursing assessment is done on the person with the neurological problem, and then am able to manage their care and refer if needed. Provide any counseling as required within the scope of nursing and advocacy as required. Provide education to the community about the various disorders and how they affect an individual. Telecross recruits and trains volunteers to provide a daily telephone call to people who live alone and are at risk of sudden health deterioration. If the volunteers' call is not answered, a Red Cross staff member arranges for someone to visit the client's home and check on their well-being. The Home and Community Care program in WA provides funding to assist Red Cross in the operation of this service. Each morning between 7.30am and 9am, trained volunteers telephone clients at a predetermined time to check on their health and wellbeing. ( need to be assessed ). Should the telephone not be answered, emergency procedures are actioned to determine if the clients is safe and well. This statewide HACC service is based in Perth and the office at Lotteries House is used for Carer Support meetings.

People Who Care is a volunteer-based, non-profit and non-denominational charity helping the frail aged and people with disabilities to continue living independently for as long as possible. This is a HACC service. South West Metro Service assists clients in the Town of Kwinana, City of Rockingham and City of Mandurah areas and surrounding areas . Services provided include Transport, gardening, shopping, social support outside of home and centre-based day care(females only at this stage, twice a week). The Guildford op shop is now located at the front of their premises, 48 James Street, Guildford. The shop carries: furniture , white goods , bric-a-brac ,electrical items clothing , beds, books and cd's. All of the goods sold by the Shop are donated. The Transport services include trips to leisure centre, community group, doctors , physios, hospital, social support, and other community transport.

Homecare Services provides social support, respite care and domestic assistance services to HACC eligible clients. We focus on services to people with a mental illness.The Health Access Program (HAP) is designed to assist people with disability to better access health care for physical illness. Homecare Services provides the Health Access Program as a social support service under the Department of Health's Depts HACC program. Our current focus is on assisting people with mental illness / psychiatric disability. The HAP can assist clients with the following; - attending the Street Doctor -obtaining a General Practitioner (GP) - making appointments with their GP - assistance in attending appointments - assistance with follow ups eg attending prescribed diagnostic testing, scans etc - providing a reminder service about appointments Please note that clients of the HAP must require more than just transport assistance.
